By creating an account, I confirm the following:
I have a licensed prescriber (physician, nurse practitioner, or physician assistant) for any prescription medication I track in Weight Commit.
I understand that Weight Commit is a logbook, not a medical service. The product does not provide medical advice, diagnosis, dosing recommendations, titration plans, treatment decisions, or any other form of clinical decision-making.
All decisions about my medication, dose, schedule, side-effect response, and care plan come from my licensed prescriber and from no one else. The data I see in Weight Commit (charts, trends, summaries, side-effect logs, dose history) is for my own self-tracking. It is not a clinical assessment, screening, or diagnostic tool. It is not a substitute for a relationship with a qualified healthcare provider.
For any decision about my medication or my health, I will contact my prescriber.
If I lose access to a prescriber, I will stop using Weight Commit to track that medication until I am again under a prescriber's care.
I understand that misrepresenting my prescriber relationship is a violation of these terms and grounds for account termination.
